TITLE: Criminal Background Checks

Criminal background checks are conducted on all employees as required by applicable law.

New employees are required to complete the Washington State Background Authorization form which is used to complete the on-line version with the Background Check Central Unit.

Newly hired caregivers are required to have a follow-up FBI fingerprint check upon Evergreen Estates receiving the acceptable results from the Background Check Central Unit. FBI fingerprint check appointments are setup by the Administrator with the employee.

Employees who report any type of criminal history on their Background Check Authorization form will NOT be hired until a determination is received by the Background Check Central Unit and the Administrator.

Employees who fail to show up for the scheduled FBI fingerprint check appointment will be given a 2nd opportunity to accomplish this task successfully. Failing to have the fingerprint process completed the 2nd time will result in the employee being terminated.

The employee will be informed of the results of all criminal background checks as required by applicable law, and the employee will document this review by signing the result form.

Employees found convicted of a crime listed on the Department of Social and Health Services issued Disqualifying List will not be hired or retained.

Every two years employees will be required to fill out another Background Authorization form, regardless of how long an employee has been employed. Evergreen Estates performs this task in December of odd years.
