TITLE: Tri-State Medical Billing Discount

ADMINISTRATIVE RESPONSIBILITIES
Evergreen is responsible for informing Tri-State on a monthly basis of those employees who are eligible for this benefit.
EMPLOYEE ELIGIBILITY DETERMINATION
Only employees occupying a full-time budgeted position and who have completed 90-days of employment are eligible for this benefit. Employees will become eligible for this benefit the 1st of the month following completion of their 90-day introductory period.
Termination of benefits will happen on the day following their last work day.
Family members and/or relatives of an employee are not eligible for the medical billing discount.
All individual financial accounts with Tri-State Hospital must remain in good standing. If Tri-State Hospital does not consider the employee’s financial account in good standing (within their policy standards) the discount will not be applied.
DISCOUNT
The amount of the discount being offered will be determined by Tri-State Hospital and may change dependent on their policies. Evergreen has no input into the amount of the discount being offered.
Upon the employee receiving their first statement from Tri-State, the employee will need to contact the billing department and verbally requesting their employee discount. Tri-State will verify the eligibility of the employee to receive the discount.
This discount will be applied to an employee’s remaining balance after all relative insurance policies have paid.
Discounts do not apply to any fees that are due at the time of an appointment, such as co-pays.
Discounts do not apply to when an employee visits any health care provider, lab, specialist or medical agency not under the control of Tri-State.
EXCLUSIONS: Any service defined as a non-covered benefit by the Medical Insurance Policy is not eligible for the discount.
