TITLE: Health Insurance Qualifications

Evergreen Estates reserves the right to change this benefit as outlined, along with changing the type of health insurance benefit being offered and how much the employee will be required to pay for the benefit.

Only FT employees who work an average of 30-hours per week will be offered this benefit. The employee must maintain a minimum average of 30-hours per week that may include PTO over a quarterly basis to be eligible for this benefit.

If the employee drops below the 30-hours per week average over a quarterly basis the employee may be removed from the plan. If the employee has 2 quarters of working below 30 hours per week over the course of a 4 quarters period of time the employee’s status will be converted to PT and they will lose all benefits associated with that of a FT employee.

Health insurance takes effect on the 1st of the month following their 60th day of employment, if they elect the insurance coverage offered by Evergreen Estates.
